THE TYPE OF INFORMATION I COLLECT FROM YOU

I currently collect and process the following information (the “personal information”):

• personal identifiers, contacts and characteristics (for example, name and contact details);

• a completed personal health plan used to assess, amongst other things, where you are now and where you want to be, educational material completed by you at various stages of the integrative health coaching process and any applicable feedback forms;

• any notes produced by me as your integrative health coach during or after our sessions together; and

• information about your usage of my website (see paragraph below headed “Cookies”) as well as information about you from e-mails you send me.

COOKIES

Cookies are small amounts of information which I store on your computer. Unless you have indicated your objection when disclosing your details to me, my system will issue cookies to your computer when you log on to www.wellmybeing.com or www.wellmybeing.co.uk. Cookies make it easier for you to log on to and use the site during future visits. They also allow me to monitor website traffic and to personalise the content of the site for you. You may set up your computer to reject cookies although, in that case, you may not be able to use certain features on my site. If you do not wish to receive cookies in the future, please let me know.

HOW I GET THE INFORMATION AND WHY I HAVE IT

Most of the personal information I process is provided to me directly by you, as it will enable me to supply you with the integrative health coaching services you have requested.

Under the General Data Protection Regulation (GDPR), the lawful basis I rely on for processing this personal information is your consent. You are able to remove your consent at any time. You can do this by contacting me by e-mail: faiz@wellmybeing.com. The condition for processing special category (information about an individual’s health) data is satisfied by explicit consent under Article 9(2)(a) of the GDPR.

WHAT I DO WITH THE INFORMATION I HAVE

I use the personal information that you have given me in order to:

• provide a framework for you to explore your health goals and successfully achieve them;

• contact you, where necessary, about our integrative health coaching sessions; and

• use and analyse the information I collect so that I can administer, support, improve and develop my integrative health coaching business.

I may also use the personal information to let you know about any promotional offers, news, events or other services that may be of interest to you and I may contact you by telephone as well as by e-mail. If you change your mind about being contacted in the future, please let me know.

I may share your personal information with third parties who provide technical support, content management, web analytics, search engine optimisation, marketing or other similar types of services to WellmyBeing. I may also pass aggregate information on the usage of my website to third parties but this will not include information that can be used to identify you.

HOW I STORE YOUR INFORMATION

The personal information you provide me with is securely held in a filing cabinet or on my computer in the United Kingdom. I may also use a secure content management system to store your personal information. I will keep your personal information for a period of five (5) years after which it will be destroyed or permanently erased (as the case may be).

YOUR DATA PROTECTION RIGHTS

Under data protection law, you have rights including:

• you have the right to ask me for copies of your personal information.

• you have the right to ask me to rectify information you think is inaccurate.

• you have the right to ask me to complete information you think is incomplete.

• you have the right to ask me to erase your personal information in certain circumstances.

• you have the right to ask me to restrict the processing of your information in certain circumstances.

• you have the right to object to the processing of your personal data in certain circumstances.

• you have the right to ask that I transfer the information you gave me to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you. Please contact me at faiz@wellmybeing.com if you wish to make a request. If you are not happy with how I have used your data, then please contact me in the first instance. You can also contact the Information Commissioner’s Office.

CHANGES TO PRIVACY POLICY

Any changes to my privacy policy in the future will be posted to this website and, where appropriate, through e-mail notification.
